Critical Risk Management in Offshore Oil & Gas: Controlling the Risks That Matter Most

Offshore Oil & Gas projects bring together complex activities, multiple work groups, changing conditions and potentially high-consequence hazards. In such environments, effective HSE management requires more than identifying hazards—it requires ensuring that critical controls remain effective throughout the work.

My experience across offshore fabrication, installation, hook-up and commissioning has reinforced the importance of focusing attention on the risks that can have the most serious consequences.


Start With the Work, Not Just the Checklist

Risk assessment should begin with understanding the actual work.

What is happening?What could go wrong?What could cause the event?What controls are required?Are those controls actually in place?

Activities such as lifting, confined-space entry, hot work, work at height, gas testing and SIMOPS require disciplined planning and field verification.


SIMOPS: When Activities Interact

Simultaneous operations can introduce risks that may not be obvious when individual activities are assessed separately.

One work activity can affect another through:

  • Energy sources

  • Equipment movement

  • Access and egress

  • Ignition sources

  • Gas release potential

  • Lifting operations

  • Permit conflicts

  • Emergency response requirements

Effective SIMOPS management therefore requires communication between disciplines, clear responsibilities and continuous monitoring of changing site conditions.


Permit to Work Is a Control—Not Just a Document

A PTW system is effective only when it represents the actual conditions of the work.

Permit verification, isolation requirements, gas testing, worksite checks and communication between responsible personnel are critical elements of effective control of work.

Industry process-safety frameworks identify work control, permit to work and task risk management as important components of process safety management.


Stop Work Is Part of Safety Leadership

A strong safety culture gives workers and supervisors the confidence to intervene when conditions become unsafe or uncertain.

Stop Work Authority should not be viewed as a failure of the project. It is an important intervention mechanism when the actual conditions no longer match the approved controls.


Managing Risk Through the Life of the Job

Risk management does not end when a JSA or permit is approved.

Conditions can change. Work fronts can change. Personnel can change. Weather can change. Interfaces between activities can change.

That is why effective HSE leadership requires continuous field verification and communication.

The objective is not simply to identify risk. The objective is to ensure that critical controls remain effective when the work is actually being performed.
